Hey guys, I'm an oldskool NWA fan, so by the time the Attitude Era came around, I was still trying to hold on to the downfall of WCW when the nWo angle went on far too long. And not being the biggest Undertaker fan to begin with, I think I missed his DM4 re-image.
This was the time when he formed the Ministry right? And the highlight was throwing off Mankind from the top of HITC for that huge bump correct?
So what was the significance behind the teardrop on his face? At first I thought it was a beauty mole. Did he have a story behind it? Like the Native American Indian that shed a tear for all the pollution back in the 80s?

I could be wrong but, I think either he added the teardrop because of the betrayal of Paul Bearer. Or that or it may have had something to do with him killing his brother (Kane) when he was only a child. Thus laying the ground work for his character to be brought in.

I thought the tear debuted at Survivor Series 96 which was about the time Paul Bearer had left him.

According to Paul Bearer himself, the tear drop was a tribute to someone very close to Undertaker who had suddenly passed away at that time.
However, as far as the timeline goes, Undertaker came back at Survivor Series 1996, feuded with Mankind, Executioner, Vader, and then won the WWF title from Sycho Sid. It wasn't until mid 1998 that the 2nd PPV HIAC match took place, and late 1998 that the Ministry was created.

Sean is right, it was in memory of someone who had died that was close to Callaway.

Undertaker debuted at survivor Series 1990 in the black and grey outfit ( he had black gloves to start,but would switch to grey shortly thereafter)...
SummerSlam 1994 he returned after being "killed" by Yokozuna at the Royal Rumble and debuted the black and purple "Resurrected" look....
Survivor Series 1996 he debuted the DM 4 figure look....he would go on to change and play with this look til late 1998 ,but it was usually basically the same : teardrop tattoo, black shirt,black pants,and biker gloves...occassionally there would be some brown pieces mixed in here and there to the ensemble, and most of the time he would wear a robe similar to the Elite 8 figure one,but sometime he would appear without it...
there was a "one-time" reappearance of the original black and grey outfit for his match against Sycho sid for the WWF Championship at WM 13...
in December 1998 - January 1999 he started the Ministry,and the demonic look.... this would last til August - September of 1999 when he was "fired" by Vince...
he returned in May 2000 with the "American Bad Ass" look, long hair,bandana,sunglasses ,leather pants,jean shirt , long leather coat,and biker gloves...
in late 2001 he started the heel turn and did the "Big Evil" look ,which was basically the ABA look with shorter hair ,no coat,and didn't wrestle in a shirt anymore...
in 2004 he debuted the Deadman persona once more,which he still uses to this day...
