Definitely between '95 and '97.
Besides the Mass Transit Incident, you also had the Taipei Death Match at Hardcore Heaven '95 between the Rotten Brothers, which was almost sickening to the point of distraction.
I'll give D-Von props though, whenever he's gotten colour, it's been incredibly plentiful.

95 was definitely the most violent, if simply for the Rotten's feud. No other mainstream American wrestling federation had the balls to do barbed wire baseball bats, glass fists, and thumbtacks, especially in 1995. They had a series of these matches, in addition to some bat matches & barbed wire bat matches with both the Pitbulls and the Public Enemy. Plus The Pitbulls had some real bloodfest dog collar matches with Raven/Richards & Public Enemy.
Mikey Whipwreck & Sandman's feud was also that year, with the most horrible "loser takes 10 lashes from the cane" match I have ever seen. Mikey takes it SO much worse than Dreamer did. Woman is completely hilarious in her acting during it to, as if she's about to climax from the violence. They had two of those matches and a ladder match that was pretty wild.
