I was just thinking about this:
Last summer, John Cena's elbow was the size of a tennis ball. He came on Raw the night after Summerslam and said he faced surgery and would have to miss at least 6 months. That would have put him out until February at the earliest, missing a good portion of the Wrestlemania build-up (not saying he wouldn't get a match, but takes away from whatever feud he was set to be in). Most importantly, however, Cena missing six months would set his return after his title unification match at TLC.
My question to you guys is: had Cena not been the Bionic Man and come back in 2 months, would the World Titles even be unified? Could they have had a unification match that was not Cena/Orton? Hell, how could Wrestlemania play out with either two World Titles or without a 3-month-long Cena/Wyatt build-up?
I don't think they could have unified the titles without John Cena involved. No chance in hell we'd see Del Rio/Orton, so ADR would have to drop the belt (and not to Sandow either; no chance there). Giving Bryan the WHC would be a viable idea, but making him a 4-time World champ before his big payoff, which one way or another should be at WM, is utterly pointless. Punk wouldn't have signed off to be a transitional champ just to give Orton both belts. We would have been looking at 2 World Titles until probably Summerslam this year before a possible unification.

Well, for starters sandow's career would be a hell of a lot better.
And Barrett, ambrose, Rollins and wyatt would have a chance at a world title

Considering there's serious doubt as to whether or not Bryan's big Wrestlemania moment was ever the intention in the first place, I'd imagine Bryan would have just been involved in the unification process if they still carried it out.
Either he or Punk. They were the only two guys on the roster at the time that had the kind of star power necessary for such a match-up.

At the time, the only 4 big names you had were Cena/Orton/Punk/Bryan.
I think they would have went through with the unification process either way, it just would have involved either Punk or Bryan.

Orton would probably have dropped to Bryan at WrestleMania involving Batista, after beating H. The belts got unified at Royal Rumble, ADR vs Orton, no matter how much you dislike the idea.
Punk would STILL be gone and getting talked about everyday.
Bray would have most likely have still feuded with Cena since there feud started after Royal Rumble pre Elimination Chamber.
So basically, nothing would have really changed drastically.

And Cena is a cancer anyway, so it probably does take 4 guys out of the title picture when he came back.

Think about it, no way does Barrett or any of the others I listed ever hold the main belt when there is only one. And Cena is a cancer anyway, so it probably does take 4 guys out of the title picture when he came back. Over time yeah, but it's not like Cena woulda been out long enough for that to happen. He was gonna be out for 6 months, not a year or more, and honestly Cena being there or not wouldn't have affected those four's standings at the time. 0% chance with or without Cena that Barrett or Ambrose or Rollins or Wyatt woulda won the title or been close to the title between Summerslam and Royal Rumble, because none of them were in a position to even get close to it, all of them were doing something completely separate, and with Orton and Bryan and Punk there at the time, they wouldn't have been made close enough, and Cena winning at MITB doesn't effect them either, because Rollins and Ambrose are doing their thing and Wyatt is doing something with Jericho and Barrett's hurt anyway, and Cena's 2 month reign to drop the belt to Brock isn't gonna kill anybody, beyond popular belief. You're right that Barrett and Ambrose and Rollins probably have no chance of winning the title with only one now, but it's not like that's Cena's doing or his fault so that's completely irrelevant. There's only so many people that can go for the title, and that's not Cena or Cena's fault, that's just schematics. And if Cena is a cancer, that's a damn good money making cancer I'd love for my company to have.

I would say they would have went with the unification anyways, just at WrestleMania. They could have still done the thing with Vickie telling whoever was World Heavyweight Champion that at either Royal Rumble or Elimination Chamber they would face John Cena in his return match for the title. Sandow might have cashed in earlier and still lost and at the end of Elimination Chamber after Orton won, do the whole John Cena comes to the ring to confront Orton at the end, and that could have been the Wrestlemania main event. Not saying I would have liked that, cause I might have turned of the Network as soon as the match would have started but I would have seen the unification happening if Cena would have stayed away longer from his injury.

Their most over face gets injured so they put the belts on someone who is a draw. Simple as that.
