I would guess it depends on who it is.
I doubt Curt Hawkins had any input in his figure, but I'm sure that if CM Punk or John Cena came to them with a suggestion, they would at least take it into consideration.

A wrestler could stick to one attire for 3-6 months which would almost force Mattel to make that design. Otherwise, I don't see too many wrestlers having any say.

Some do and some don't,I'm sure. Just like every other decision in wrestling.
I know that Zack Ryder has worked closely with Mattel ever since he was disappointed with the first figure. But I'm sure most guys don't care beyond signing off on likeness.

one or two of them mite get a say in what it looks like or what attire they would rather have, but im still guessing its down to the MDT and WWE what they end up bring out

I know in the past some have. Saturn for example was able to get his Jakks fig in trunks because it was his preference, and Tazz was pretty pissed off with how one of his turned out, so they made the Ringside exclusive, and they claimed that Harley Race nixed a fig with the moustache, but I more believe that was just Jakks being lazy and putting the heat on Race. Maybe its different at Mattel. I don't know.
