Without a doubt. No movie has terrified me more than the Texas Chainsaw Massacre to this day. I couldn't even watch it all the way through the first time. And what I love about it, is that it is in daylight alot of the time and it is still scary.

Probably Terminator 3, I was scared that such a bad film had been made.

I know a lot of people disagree with this, or even flat out hated the movie...
Paranormal Activity.
Nothing, ever, has even come close to bothering me the way that movie did and still, to this day, does. I was never the type of person to be freaked out or bothered by horror movies. I could watch pretty much anything and sleep soundly at night, never to think of it again. Paranormal Activity changed that. Not only did it keep me awake for days, it weakened my mental psyche when it comes to any and all horror movies. Films I could have watched without even flinching five years ago now drive me insane.
No film has ever had that effect on me before, and I strongly doubt another ever will.
I'm absolutely terrified of what I can't see, and that's what made me so uncomfortable when watching Paranormal Activity. Everything felt so real, so menacing.
I saw the movie twice in theaters and haven't been able to watch it again since. I own both films, primarily out of respect to the filmmakers, as I doubt I'll ever watch them again.
The other thing that absolutely terrifies me is the Marble Hornets Slender Man series on YouTube. I'll tell you, a few guys with a camera and a knack for story-telling made a far more compelling saga than nearly any horror movie franchise could dream of doing. It's terrifying, so much so that I can't even continue watching them.

Gotta go with The Exorcist (1973).

The Shining...... until I saw Insidious, say what you will about the film, but it scared the ing crap out of me like no movie ever has before. I was actually afraid to go home alone after the movie, and didn't sleep the entire night after watching it. Truly terrified me.

The first Scream, I saw it when I was around the age of 4. Haven't seen a scary movie since.

The Shining.
An amazing film and one of my all-time favorite, but creeped me out more than any film ever made. It gets scarier everytime.

John Carpenter's Halloween.
Another movie that scared the hell out of me as a kid is Silent Night, Deadly Night. From what I have been told, after watching that movie I was terrified of Santa. Apparently my memory has blocked it out because I do not remember ever seeing it.

As a kid, Darkness Falls. Pitch Black, surround sound, and no one but me and a friend the same age, yeah not a good combo. Now though nothing does.

jeepers creepers actually freaked me out for a few days when i saw it when i was 12, but about the only other horror movie to legitimately freak me out was the original Halloween. which is still one of my favorite movies to this day
